Generally, for charges filed to insurance, these are the steps you can expect:

Step 1:

  • You estimated financial responsibility is due before treatment is rendered.
  • We collect a $200 deposit to schedule your surgery appointment. ( Implant surgeries will pay 50% deposit at time of scheduling)
  • Many patients prefer to pay their estimated portion immediately, but we are happy to contact you the week before your appointment for payment on the balance of your estimated portion.

Step 2:

  • Insurance processes the claim and issues payment, if applicable.

Step 3:

  • Your payment of any remaining is due or we may owe your a refund. You will either receive a statement or refund from us. 

For your convenience, we accept Visa, MasterCard, Discover, American Express and Care Credit. We also accept checks and cash.  Payment is due at the time of service. If you have questions regarding your account, please contact us at Lakewood, WA Phone Number 253-584-0858. 

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a statement for any remaining balance after your insurance has paid their portion. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to the actual procedure.
